Step back in time with this rare and original 1918 Order Form for Opium, Etc., documenting the purchase of cocaine by W.A. Lane, D.D.S., a dentist from Tallassee, Alabama, dated July 24, 1918.

Order forms like this one were issued during the time of the Harrison Narcotics Tax Act (1914), which regulated opiates and coca products. It stands as a tangible piece of early 20th-century medical and pharmaceutical history, showcasing the era when regulated substances like cocaine were legally ordered for professional use — before evolving laws reshaped the landscape of medicine and highly addictive controlled substances.

This form comes framed in a high-quality wooden frame for its protection. As is obvious from the photos, it became torn at the top and was subject to some wear over the past 107 years. This only adds to its historic charm and authenticity!

It would look great as office decor or hanging in a bar!
